FTE Classification: 37.5 hours/week, FTE Non-Exempt

Report To: Executive Director

Location: Fresno County

*Job Announcement: *West Fresno Family Resource Center is seeking a
qualified candidate to implement the collaborative Families Together
program. The program will provide prevention and intervention services to
effectively meet the needs of children and families at risk of child
abuse and/or
neglect. The program will increase safety, strengthen families and build
strong and healthy communities by targeting these families and building
coping strategies to encourage a state of well-being that allows the
individual/family to function in challenging circumstances and prevent children
from entering the foster care system.

*Position Summary: *The Case Manager will address underserved populations
in 93706/93722 with the goal of strengthening families and communities
through case management services to families referred by Child Welfare
System. The case manager will conduct a family needs assessment, conduct
internal, external and cross referrals and link clients to parent education
classes, peer-to-peer support. The case manager will maintain case
management for 6 months which includes home visitation, one-on-one
appointments and follow-up assessments.

*Qualifications: *Candidate must possess excellent written and verbal
communication, ability to follow-through, and be detail-oriented. Candidate
must be organized, professional, and able to work in a fast-paced,
multi-faceted environment. Candidate must be proficient in the use of
technology to collect, analyze and manage program data as well as computer
literate in Windows platform; proficient in Microsoft Word, Excel and
Google Applications. Candidate must possess the ability to maintain good
recordkeeping and documentation, good presentation skills, and have the
capacity to be self-motivated. It is mandatory to meet employment
eligibility requirements as established by the program’s funding agency.

*Minimum Education: *Bachelors’ degree in social work, human service, or
related field.

*Minimum Experience:* Two years’ experience in a community based human
service agency providing direct service to clients as a case manager,
social worker, advocate, or other related work title. Candidate must
possess knowledge or familiarity with the child welfare system and community
resources to provide linkages and navigation.
